The following Tampa Bay area restaurants received the most violations during inspections conducted Nov. 12 to Nov. 16.
Taco Bus Inc. at 913 E. Hillsborough Ave. in Tampa, 22 violations
Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included: rice 78F°, cooked chicken 55F°, cooked beef 70-86F°, Pico 45F°, shredded cheese 48F°, cut leafy greens 54,47F° and cooked ground beef 50F°.
- The cutting boards were heavily stained and soiled.
- A toxic chemical, bleach was stored by faux butter.
Cody’s Original Roadhouse at 3101 Commercial Way in Spring Hill, 26 violations
- Potentially hazardous hot food was held at less than 135 degrees Fahrenheit or above. This included: chicken breast 129°F, soup 114°F, cooked mushrooms 117°F, baked potato 126°F and prime rib 98°F.
- Raw chicken was not properly separated from beef and pork in the holding unit.
- There was evidence of employee smoking in the food storage area. An ashtray with cigarette butts was found in the storage area across from the freezers.
New York Buffet at 8503 W. Hillsborough Ave. in Tampa, 23 violations
- There was an accumulation of black/green mold-like substance in the interior of the ice machine.
- Ready-to-eat, potentially hazardous food was prepared on site and held for more than 24 hours without being properly date marked.
- A case of chicken was stored on the floor in the walk-in cooler.
Danny’s All American Café at 4406 Falkenburg Rd. in Tampa, 22 violations
- Live, small flying insects were found in the kitchen, food preparation area, and food storage area. 2 flies were on the front line, 1 on the plastic wrap box, 1 on top of the reach in freezer, 4 to 5 in the kitchen, approximately 6 to 8 near the three compartment sink, 1 on the trash can in the kitchen, 1 on the napkins near ice cream freezer, 3 near the ice cream area, 2 dead flies were found in the soda reach in cooler, 1 on the single service containers near the ice cream cooler and 2 live flies were found on the tongs hanging on the rack.
- Roach activity present as evidenced by live roaches found. 1 live on the door of the reach in cooler, approximately 5 on the back of the shelf next to sandwich reach in cooler, 1 on the wooden sneeze guard above the sandwich reach in cooler and 1 behind the soda reach in cooler.
- Dead roaches were found on premises. 3 on top of the hot water heater, 1 on the drain boards above the three compartment sink, 1 on the door of the two reach in cooler, 1 under the table where the slicer is located, 5 dead on the electrical outlet, 1 on gasket of the reach in freezer, 1 on the floor at the entrance, 1 above the drink cooler and approximately 5 to 8 dead roaches in a wall crevices near the ice cream freezer.
Thai Basil at 4445 E. Bay Dr. in Clearwater, 24 violations
- Raw chicken was not properly separated from vegetables based upon minimum required cooking temperature in the reach-in freezer.
- Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included garlic/oil mix 83°F and cut lettuce 73°F.
- Unwashed mushrooms and cabbage were stored with ready-to-eat cut vegetables in the walk-in cooler.
Mori Sushi Inc. at 1136 94TH Ave. N. in St. Petersburg, 31 violations
- Nonexempt fish was offered raw or undercooked that has not undergone proper parasite destruction. Fish must be fully cooked or discarded. This included: conch, mackerel and smoke salmon. The operator was unable to provide documentation that the aqua-cultured fish has been raised in a controlled environment and fed formulated feed.
- Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included: raw shrimp 46°F, raw chicken 47°F, raw shrimp 49°F, Krab 58°F, cream cheese 53°F, cooked eel 58°F and raw fish 62°.
- Roach excrement and droppings were found on the wall by reach in freezer, between the wire shelf, the wall in dish area and kitchen.
Manatees on the Bay at 3128 Beach Blvd. S. in Gulfport, 25 violations
- An employee failed to wash their hands after drinking from an uncovered plastic cup and then serving patrons.
- A puddle of soiled mop water was on the ground outside of the kitchen door.
- The floor was soiled with an accumulation of debris under the bar areas.
Gill Dawg at 5419 Treadway Dr. in Port Richey, 26 violations
- Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included: slaw mix 46°F, bleu cheese 46°F, whole mozzarella 46°F, cut tomatoes 45°F and rice 45°F.
- The cutting board had deep cut marks and is no longer cleanable.
- The wall was soiled with an accumulation of black debris in the dishwashing area.
