The following Tampa Bay area restaurants received the most violations during inspections conducted from August 20 to August 25.

Chill Restaurant at 357-363 Corey Ave. in St. Pete Beach, 32 violations

  • There was no proof of parasite destruction or aquaculture documentation for cold smoked salmon served raw or undercooked. The fish must be fully cooked or discarded.

  • The wall near the cook-line was soiled with accumulated grease, food debris, and dust.

  • Raw tuna in the cooler drawers was held at greater than 41 degrees Fahrenheit.

China Wok at 2143 Collier Parkway in Land O Lakes, 30 violations

  • Raw chicken was stored over raw beef. The beef was stored over mushrooms. Fresh eggs were stored over ready-to-eat sauce and repackaged chicken was over beef.
  • Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included: sprouts 80°, garlic in oil 86°F, cabbage 82°F, egg rolls 82°F and rice 76°F.
  • An employee touched soiled apron and then engaged in food preparation without washing their hands.

Hooks On 4th INC at 7900 4th St North in St. Petersburg, 28 violations

  • Nonexempt fish was offered raw or undercooked that has not undergone proper parasite destruction. Fish must be fully cooked or discarded.
  • Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included: bean sprouts 50°F, raw shrimp 44°F, raw fish 44°F, raw beef 45°F, raw shell eggs 44°F, krab 44°F, cream cheese 44°F, kitchen 46°F and cut lettuce 60°F.
  • An accumulation of black/green mold-like substance was found inside the ice bin.

Amicis Catered Cuisine at 205 Marlborough St. in Oldsmar, 32 violations

  • Raw vegetables were not washed prior to preparation.
  • An accumulation of black/green mold-like substance was found in the interior of the ice machine.
  • A buildup of food debris and soil residue was found on equipment door handles.

DJ Mori Sushi LLC at 1136 94 N in St. Petersburg, 27 violations

  • Nonexempt fish was offered raw or undercooked that has not undergone proper parasite destruction. Fish must be fully cooked or discarded.
  • Raw fish was stored over ready-to-eat sauces and vegetables.
  • Roach excrement and droppings were found in the cabinet at the server station and the sushi bar.

Steak N Shake Inc #274 at 768 E SR 44 in Wildwood, 31 violations

  • Raw animal foods were not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. This included raw ground beef over pooled.
  • Approximately 20 to 25 dead flies were found on fly strip in the office.
  • An accumulation of food debris and grease was found on the liquid cheese dispenser.

Babalu Restaurant & Full Bar at 9246 4th St N in St. Petersburg, 24 violations

  • Potentially hazardous cold food was held at greater than 41 degrees Fahrenheit. This included: tuna salad 50°F, coleslaw 51°F, chicken 47°F, fish spread 45°F and butter 45°F.
  • Raw animal foods were not properly separated from one another based upon minimum required cooking temperature in walk-in cooler. This included raw chicken over raw shell eggs.
  • The perimeter walls and roof of the storage closet do not effectively protect the establishment against environmental cross-contamination or the entrance of pests.


China Gourmet III at 343 Colony Blvd. Ste. 108 in The Villages, 35 violations

  • An employee handled soiled dishes or utensils and then handled clean dishes or utensils without washing their hands.
  • Potentially hazardous food was held at greater than 41 degrees Fahrenheit. This included fried chicken cooked and stored at room temperature of 85°F.
  • Ready-to-eat, potentially hazardous food was prepared on site and held more than 24 hours without being properly date marked. This included: chicken strips, chicken wings, raw ground beef and raw shrimp.
