Restaurant Ratings Most Violations: June 16 - 21

Ever wonder what goes on in the kitchen of your favorite restaurants?

We’ve gathered details about the 30 Tampa Bay restaurants that received the most violations during inspections conducted from June 16 to 21, 2014.

Each restaurant is shown in the slideshow above. The total number of violations the restaurant received is shown in the plate on the left side of the slide. Some details from the inspection report are listed below the slide. If you click on the graphic in the slideshow, it will take you to the original state inspection report.

The 8 restaurants with the highest number of violations for the week are listed below.

Buffet City at 13235 Cortez Blvd. in Brooksville, 63 total violations

State inspectors visited on June 20 and found an accumulation of flies in trapping devices.

They also found one live roach in the kitchen area.

Inspectors saw an accumulation of a black or green mold-like substance inside the ice bin.

During the previous inspection on February 2, 2014, inspectors found 7 total violations.

El Caballero De Los Mares at 2104 W. Busch Blvd. in Tampa, 50 total violations

State inspectors visited on June 17 and found fish, which is a potentially hazardous food, being thawed at room temperature.

The establishment was also operating without a license from the Division of Hotels and Restaurants.

Inspectors also saw live flies in the kitchen. Shell eggs were held at room temperature with an ambient air temperature greater than 45 degrees and raw beef and shrimp were being held at greater than 41 degrees.

There is no previous inspection record because the restaurant was operating without a license.

China Wok at 35611 U.S. 19 in Palm Harbor, 47 total violations

State inspectors visited on June 17 and say a screen door was torn and in poor repair and vermin were also present.

Equipment was in poor repair. Some bus tubs that were being used to store food were cracked and broken.

Flour, sugar, MSG, dry rice, and bread crumbs were stored in dry storage area but were not covered.

Nonfood-grade plastic to-go/thank you bags were being used to store food.

Frozen beef chunks were found unwrapped in a chest freezer.

Food-contact surfaces were not sanitized after cleaning or before use.

During the previous inspection on May 6, 2014, inspectors found 7 violations.

Del Valle Latin Store & Market at 1520 East Fowler Ave. in Tampa, 43 total violations

State inspectors visited on June 19 and found 3 dead roaches in a cabinet in the front counter in the restaurant's bakery area. They found one live roach in the bakery and one live roach on the cook's line.

An employee was preparing food but was not wearing a hair restraint.

Food was stored on the floor.

Inspectors say a bar area was added to the restaurant, but a plan review was not submitted or approved.

During the previous inspection on March 27, 2014, inspectors found 29 violations.

Lagerheadz Bar & Grill at 606 N. Pinellas Ave. in Tarpon Springs, 42 total violations

State inspectors visited on June 19 and found 3 dead roaches in the kitchen and prep area. There were also small flying insects in the bar and kitchen area.

Inspectors found a can of bug spray stored next to some wine.

They also say a wall was soiled with accumulated mold or mildew.

During the previous inspection on December 3, 2013, inspectors found 12 violations.

Ivory Mandarin Bistro at 2192 Main St. in Dunedin, 40 total violations

State inspectors visited on June 18 and found dead roaches on the premises. They spotted 4 dead roaches in a storage room, 5 more were in a corner outside of the walk-in cooler.

A cook was preparing food, but was not wearing a hair restraint.

All potentially hazardous cold foods held in a reach-in cooler were greater than 41 degrees.

A wall was soiled with accumulated food debris.

During the previous inspection on May 29, 2014, inspectors found 7 violations.

Ichiban Japanese Culture at 2786 Fowler Ave. in Tampa, 37 total violations

State inspectors visited on June 20 and say an employee's personal food, chicken wings, were not properly identified and segregated from food to be served to the public.

Raw fish was held at temperatures greater than 41 degrees. Fish was at 42°f, shrimp was 36°f and tuna was 42°f.

During the previous inspection on February 27, 2014, inspectors found 25 violations.

George's Breakfast Station at 600 South Pinellas in Tarpon Springs, 36 total violations

State inspectors visited on June 20 and say an employee was preparing food without wearing a hair restraint.

Bleach was stored on a shelf with sugar and oil.

There were 3 cases of eggs stored on the floor. Eggs were being stored at room temperature with an ambient air temperature greater than 45 degrees.

During the previous inspection on June 9, inspectors found 25 violations

Inspectors from Florida's Department of Business and Professional Regulation conducted the inspections. The department cited violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code.

While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary, according to the Florida DBPR website.
