Restaurant Ratings Most Violations: April 21-27

Ever wonder what goes on in the kitchen of your favorite restaurants?

We’ve gathered details about the 30 Tampa Bay restaurants that received the most violations during inspections conducted from April 21 to 27, 2014.

Each restaurant is shown in the slideshow above. The total number of violations the restaurant received is shown in the plate on the left side of the slide. Some details from the inspection report are listed below the slide. If you click on the graphic in the slideshow, it will take you to the original state inspection report.

The 8 restaurants with the highest number of violations for the week are listed below-


1) Steak n' Shake at 10555 Ulmerton Rd. in Largo, 44 total violations

State inspectors visited on April 22, 2014 and found turkey slices & cottage cheese held at an unsafe temperature above 41 degrees. Inspectors issued a stop sale on the food items due to temperature abuse. Inspectors also found an accumulation of an orange mold-like substance in the ice machine.

During the previous inspection on November 6, 2013, inspectors found 14 violations.

2) Dragon Express at 2101 E. Fletcher Ave. in Tampa, 43 total violations

State inspectors visited on April 22, 2014 and found problems with roaches in the restaurant. Live, small flying insects were in the food storage area. Inspectors also say they found three live roaches in sticky traps in the dish room. They discovered dead roaches, too. One dead roach was in a sticky trap on the cook's line. Three dead roaches were on sticky traps in the dish room. One dead roach was found in a sticky trap in the dry storage area.

During the previous inspection on December 16, 2014, inspectors found 34 violations.

3) Persia House of Kabob at 1441 E. Fletcher Ave. in Tampa, 36 total violations

State inspectors visited on April 22, 2014 and found problems with rodents and bugs. Inspectors say they discovered more than 20 dead roaches behind a freezer. Five dead roaches were under a dry storage shelf. Two dead roaches were by a front counter.

Inspectors say there was evidence of live roaches at the restaurant, too. One roach was on wire shelving in the dry storage area. One roach was under shelving in the dry storage area. One roach was in front of a walk-in cooler by dry storage. Four roaches were behind a freezer in the dry storage area. Nine roaches were on an acoustic tile in dry storage. One roach was by the steam buffet tables.

Inspectors also found evidence of rodent activity. More than 10 rodent droppings were found under a front counter. Rodent rub marks were also along walls or ceilings. Rodent gnaw marks were spotted along a wall in dry storage.

Inspectors say they saw a spider under a three-compartment sink.

During the previous inspection on December 16, 2014, inspectors found 24 violations.

4) Koi Sushi at 7900 4th St. N. in St. Petersburg, 36 total violations

State inspectors visited on April 22, 2014 and found too many live flies in the kitchen to count. They also say a toxic substance or chemical was next to a flat top grill. The chemicals were bleach, Pine Sol and a purple liquid. Inspectors also found an accumulation of a black and green mold-like substance inside the ice bin.

During the previous inspection on March 6, 2014, inspectors found no violations.

5) China Latina at 3715 E. Busch Blvd. in Tampa, 36 total violations

State inspectors visited on April 24, 2014 and found raw animal food stored over ready-to-eat food, which could cause cross contamination. Raw meat was stored over uncovered sauces. Inspectors also say the interior of the microwave was soiled with encrusted food debris. Stored food was not covered in the restaurant's walk-in cooler.

During the previous inspection on July 15, 2013, inspectors found 20 violations.

6) Eve's Family Restaurant at 3150 Tampa Rd. in Oldsmar, 34 total violations

State inspectors visited on April 25, 2014 and said that the dishwasher handled dirty dishes or utensils and then handled clean dishes or utensils without washing his/her hands. The interior of a microwave was soiled with encrusted food debris. Inspectors say they saw an employee not using hand antiseptic after washing their hands as required.

During the previous inspection on October 1, 2013, inspectors found 10 violations.

7) Golden Bowl Chinese Restaurant at 3447 Lithia Pinecrest Rd. in Valrico, 32 total violations

State inspectors visited on April 21, 2014 and found a product containing fresh garlic held at greater than 41 degrees. They also said that potentially hazardous cold food was stored at greater than 41 degrees. Inspectors say they saw a fly in a sticky tape that was hanging over a prep table.

During the previous inspection on October 30, 2013, inspectors found 27 violations.

8) Latin Grill at 3318 John Moore Rd. in Brandon, 32 total violations

State inspectors visited on April 23, 2014 and found that an employee was preparing food, but was not wearing a hair restraint. There were also small flying insects in a food storage area and small ants on a bag inside a box. Inspectors say they found an accumulation of a black and green mold-like substance inside a ice bin.

During the previous inspection on November 22, 2013, inspectors found 23 violations.

Inspectors from Florida's Department of Business and Professional Regulation conducted the inspections. The department cited violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code.

While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary, according to the Florida DBPR website.
